Virker for mig som om at du har kopieret /var/lib/mysql/mysql fra en
3.et.eller.andet og den nye er en 4-version.
 
Jeg havde selv problemet i ovenstående tilfælde.

Løsningen er at af-installere og fjerne /var/lib/mysql og derefter
installere igen og så lade være med at overskrive denne base, eller evt.
benytte mysqldump til de gamle og derefter importere UDEN at ændre
tabelstruktur.

Jeg har sat en ny debian server op og mysql virker fint. Men når jeg 
kører "/etc/init.d/mysql restart" får jeg:

sslug@sslug root]# /usr/bin/mysqladmin: connect to server at 
'localhost' failed
error: 'Access denied for user: 'sslug@sslug' (Using 
password: YES)'

Så vidt jeg kan forstå på google er det noget med at debian-sys-maint er 
den mysql bruger som kan genstarte mysql. Men jeg har ikke fundet nogen 
forklaring (jeg turde følge) på hvordan jeg får fixet ovenstående problem...
